Q4: How can a client pay an outstanding VAT charge?

No, adding lamination to the cover doesn't cause these to require VAT.
There are 3 jobs on this order with the same artwork, only one has been flagged for VAT. Is that right?
If all of the jobs are the same artwork, and rightly need VAT, then the other jobs just may have not been scanned through yet. You should check all jobs and take VAT for all jobs that will need it.
